Introducción: El neumotórax espontáneo es la causa más frecuente de ingreso urgente en los servicios de cirugía torácica. Objetivo: Caracterizar a pacientes ingresados con diagnóstico de neumotórax espontáneo. Métodos: Se realizó un estudio descriptivo de 93 pacientes con diagnóstico clínico y radiológico de neumotórax espontáneo, se establecieron las variables del estudio y se utilizaron frecuencias absolutas y porcentajes. Para la asociación de las variables se empleó el estadígrafo ji cuadrado con un nivel de confiabilidad del 95 por ciento. Resultados: Predominó el neumotórax espontáneo primario (65,5 por ciento), el sexo masculino fue el más afectado (80,6 por ciento), con mayor frecuencia en el hemitórax derecho (72,2 por ciento) y el tabaquismo como antecedente (83,9 por ciento). La pleurostomía mínima fue el tratamiento definitivo en el 72 por ciento de los pacientes. La complicación más frecuente después de la pleurostomía, fue la fuga persistente de aire. El tratamiento quirúrgico con pleurodesis mecánica, ofreció un 100 por ciento de efectividad. La mortalidad quirúrgica fue nula. Conclusiones: El neumotórax espontáneo predomina en el sexo masculino en una proporción de 4,2:1, en pacientes menores de 40 años de edad. El antecedente patológico personal que más se asocia es la enfermedad pulmonar obstructiva crónica. El síntoma predominante es el dolor torácico. El neumotórax espontáneo primario fue más frecuente y el hemitórax derecho el más afectado. El tabaquismo está presente como antecedente en ambos tipos de neumotórax espontáneo. La modalidad de tratamiento más utilizada es la pleurostomía mínima(AU)


Introduction: Spontaneous pneumothorax is the most frequent cause of urgent admission to thoracic surgery services. Objective: To characterize patients admitted with a diagnosis of spontaneous pneumothorax. Methods: A descriptive study of 93 patients with a clinical and radiological diagnosis of spontaneous pneumothorax was carried out, the study variables were established and absolute frequencies and percentages were used. For the association of the variables, the chi square statistic was used with a confidence level of 95 percent. Results: Primary spontaneous pneumothorax predominated (65.5 percent), the male sex was the most affected (80.6 percent), with greater frequency in the right hemithorax (72.2 percent) and smoking as the antecedent (83.9 percent). Minimal pleurostomy was the definitive treatment in 72 percent of the patients. The most frequent complication after pleurostomy was persistent air leak. Surgical treatment with mechanical pleurodesis offered 100 percent effectiveness. There was not surgical mortality. Conclusions: Spontaneous pneumothorax predominated in males in a ratio of 4.2: 1, in patients under 40 years of age. The most associated personal pathological history was chronic obstructive pulmonary disease. The predominant symptom was chest pain. Primary spontaneous pneumothorax was more frequent and the right hemithorax the most affected. Smoking was present as a history in both types of spontaneous pneumothorax. The most widely used treatment modality was minimal pleurostomy(AU)

ABSTRACT Objective: To evaluate the role of intrapleural positioning of a pleural catheter in early lung expansion and pleurodesis success in patients with recurrent malignant pleural effusion (RMPE). Methods: This was a retrospective study nested into a larger prospective cohort study including patients with RMPE recruited from a tertiary university teaching hospital between June of 2009 and September of 2014. The patients underwent pleural catheter insertion followed by bedside pleurodesis. Chest CT scans were performed twice: immediately before pleurodesis (iCT) and 30 days after pleurodesis (CT30). Catheter positioning was categorized based on iCT scans as posterolateral, anterior, fissural, and subpulmonary. We used the pleural volume on iCT scans to estimate early lung expansion and the difference between the pleural volumes on CT30 and iCT scans to evaluate radiological success of pleurodesis. Clinical pleurodesis success was defined as no need for any other pleural procedure. Results: Of the 131 eligible patients from the original study, 85 were included in this nested study (64 women; mean age: 60.74 years). Catheter tip positioning was subpulmonary in 35 patients (41%), anterior in 23 (27%), posterolateral in 17 (20%), and fissural in 10 (12%). No significant differences were found among the groups regarding early lung expansion (median residual pleural cavity = 377 mL; interquartile range: 171-722 mL; p = 0.645), radiological success of pleurodesis (median volume = 33 mL; interquartile range: −225 to 257 mL; p = 0.923), and clinical success of pleurodesis (85.8%; p = 0.676). Conclusions: Our results suggest that the position of the tip of the pleural catheter influences neither early lung expansion nor bedside pleurodesis success in patients with RMPE.


RESUMO Objetivo: Avaliar o papel do posicionamento intrapleural do cateter pleural na expansão pulmonar precoce e no sucesso da pleurodese em pacientes com derrame pleural maligno recorrente (DPMR). Métodos: Trata-se de um estudo retrospectivo aninhado em um estudo prospectivo de coorte maior com pacientes com DPMR recrutados em um hospital-escola universitário terciário entre junho de 2009 e setembro de 2014. Os pacientes foram submetidos a inserção de cateter pleural e, em seguida, pleurodese à beira do leito. A TC de tórax foi realizada duas vezes: imediatamente antes da pleurodese (TCi) e 30 dias após a pleurodese (TC30). Com base na TCi, a posição do cateter foi classificada em posterolateral, anterior, fissural e subpulmonar. Usamos o volume pleural na TCi para estimar a expansão pulmonar precoce e a diferença entre os volumes pleurais na TC30 e na TCi a fim de avaliar o sucesso radiológico da pleurodese. Considerou-se que a pleurodese teve êxito clínico quando não foi necessário realizar nenhum outro procedimento pleural. Resultados: Dos 131 pacientes elegíveis do estudo original, 85 foram incluídos neste estudo aninhado (64 mulheres; média de idade: 60,74 anos). A posição da ponta do cateter foi subpulmonar em 35 pacientes (41%), anterior em 23 (27%), posterolateral em 17 (20%) e fissural em 10 (12%). Não houve diferenças significativas entre os grupos quanto à expansão pulmonar precoce (mediana da cavidade pleural residual = 377 ml; intervalo interquartil: 171-722 ml; p = 0,645), sucesso radiológico da pleurodese (mediana do volume = 33 ml; intervalo interquartil: −225 a 257 ml; p = 0,923) e sucesso clínico da pleurodese (85,8%; p = 0,676). Conclusões: Nossos resultados sugerem que a posição da ponta do cateter pleural não influencia nem a expansão pulmonar precoce nem o sucesso da pleurodese à beira do leito em pacientes com DPMR.

ABSTRACT

PURPOSE: To evaluate macro and microscopically, changes following the use of the aqueous extract of babassu (Orbignya phalerata) in the lung parenchyma and pleura of rats. METHODS: Sixty adult male rats with average weight of 350 g, were randomized into two groups of 30 animals (experimental and control) further divided into sub-groups of 10 to be sacrificed at 48 h, 72 h and 21 days. The substance was injected into the right pleura of the animals. RESULTS: There was intense pleuropulmonary macroscopic reaction with statistically significant differences between groups respectively (p<0.05, p<0.02, p<0.03). Microscopically, no statistically significant difference was evident (p>0.05). CONCLUSION: The aqueous extract of babassu (Orbignya phalerata) was found to be highly irritating to the pleura and lung of rats, evidenced macroscopically by numerous adhesions and inflammation while no major changes were evident microscopically.

Introducción: El neumotórax espontáneo secundario (NES) es la presencia de aire en la cavidad pleural con ausencia de traumatismo, asociado a alguna enfermedad pulmonar subyacente. La pleurodesis es considerada una alternativa al tratamiento de esta patología. La fuga aérea persistente o masiva es una complicación importante del NES, donde la utilización de pleurodesis con sangre autóloga constituye una opción conveniente en pacientes con alto riesgo quirúrgico. Objetivos: Se propuso describir las características de pacientes con NES tratados con pleurodesis con sangre autóloga en Hospital Regional de Concepción. Pacientes y método: Estudio descriptivo transversal, período enero 2012 - enero 2015. Se realizó una revisión de base de datos, protocolos quirúrgicos y fichas clínicas, seguimiento clínico a 36 meses, donde se describieron características clínicas, morbimortalidad, resultados inmediatos y tardíos. Resultados: Del total de pacientes (n=7), 5 (71,4 %) fueron hombres, con edad promedio de 60,7±8,2 años. La causa del NES fue mayoritariamente enfermedad pulmonar difusa en seis pacientes (85,7 %). Se realizaron siete pleurodesis con sangre autóloga sin necesidad de repetir el procedimiento. Se observó cierre de fístula en el 100 % de los pacientes. Al seguimiento no se observó necesidad de nuevos procedimientos. Discusión: La pleurodesis con sangre autóloga es un tratamiento efectivo y seguro en el manejo de la fuga aérea persistente o masiva, con adecuados resultados inmediatos y tardíos en casos seleccionados.


Introduction: Secondary spontaneous pneumothorax (NES) is pleural air occupation without trauma associated with the presence of some underlying lung disease. Pleurodesis is considered an alternative to the treatment of this disease. Persistent or massive air leak is a major complication of NES, where the use of pleurodesis with autologous blood is an accepted option in patients with high surgical risk and air leak. Objetive: Describe characteristics of patients with NES treated with pleurodesis with autologous blood in Hospital Regional of Concepción. Patients and methods: Cross-sectional descriptive study period January 2012- January 2015. It`s made a review of Database, surgical protocols and review medical records, 36 months' clinical follow-up. Description of clinical features, morbidity and mortality, immediate and remote results. Results: Of all patients (n=7), 5 (71.4 %) were males, mean age 60.7 ± 8.2 years. The cause of NES was predominantly diffuse lung disease in 6 (85.7%) patients. Seven pleurodesis with autologous blood were performed without repeating the procedure. Fistula closure was observed in 100% of patients. In follow the need for new procedures wasn't observed. Discussion: Pleurodesis with autologous blood is an effective and safe treatment in the management of persistent or massive air leak, with immediate and remote adequate results.

Investigou-se a associação entre sexo e violência física entre parceiros íntimos. Encontrou-se prevalência de sofrer qualquer violência física (17%), violência física moderada (16,6%) e violência física grave (7,3%). Não houve diferença significativa para violência física moderada em homens e mulheres, porém, quanto mais grave o ato maior a ocorrência deste nas mulheres. Por meio de regressão logística testou-se a associação da violência com o sexo, ajustando-se às variáveis exploratórias. Mulheres de maior idade, viúvas/separadas, pobres, menos escolarizadas e pretas registram maior probabilidade de sofrer violência. Nos homens, a prevalência de violência física grave apresentou alteração significativa apenas para estado civil. Uso abusivo de álcool por mulheres representou maior chance de sofrer violência física.


This study investigated the association between gender and intimate partner physical violence. A random cluster sample was chosen as the baseline cohort population in a cross-sectional design. Lifetime prevalence rates were as follows: any physical violence (17%), moderate physical violence (16.6%), and severe physical violence (7.3%). There were no significant differences between genders in moderate physical violence, but women were more likely to suffer severe abuse. Logistic regression was used to identify associations between violence and gender, adjusting for exploratory variables. Women that were older, widowed/separated, had less schooling or lower income, and African-descendants were all more likely to have suffered intimate partner violence. Prevalence of severe physical violence experienced by men only changed significantly according to marital status. Alcohol abuse by women increased the odds of suffering physical violence.


Se investigó la asociación entre el sexo y la violencia física entre parejas íntimas. La prevalencia de sufrir violencia física (17%), la violencia física moderada (16,6%) y la violencia física grave (7,3%). No hubo diferencia significativa en la violencia física moderada en hombres y mujeres, sin embargo, más grave es la mayor ocurrencia de este hecho en las mujeres. Por regresión logística se evaluó la relación entre la violencia y el sexo, y el ajuste de las variables exploratorias. Las mujeres mayores de edad, viudas/separadas, más pobres, menos educadas y negras son más propensas a sufrir violencia. En los hombres, la prevalencia de la violencia física grave cambió significativamente sólo con el estado civil. El abuso de alcohol por mujeres representan un mayor riesgo de sufrir violencia.

INTRODUCCIÓN: El derrame pleural neoplásico o maligno (DPN) es una complicación frecuente de las neoplasias metastásicas. La pleurodesis con talco es un tratamiento paliativo local efectivo, que tiene por objetivo mejorar la calidad de vida del paciente. OBJETIVOS: Describir características de pacientes con DPN y los resultados inmediatos y alejados del tratamiento quirúrgico con pleurodesis con talco. MATERIALES Y MÉTODO: Estudio descriptivo transversal, se describieron características clínicas, morbimortalidad, resultados inmediatos y alejados de pacientes con DPN tratados con pleurodesis con talco, desde enero del 2011 hasta noviembre del 2015, en Hospital Clínico de Concepción. Datos extraídos de protocolos quirúrgicos, fichas clínicas y registro de anatomía patológica, previa aprobación por el comité ético-científico. RESULTADOS: Total 70 pacientes; 47 (67,1%) mujeres, edad promedio 60,5±14,3 años (Rango 15- 85). Síntomas: disnea 67 (95,7%), dolor torácico 50 (71,4%); 43 (61,4%) correspondieron al lado derecho; 55 (78,6%) se clasificaron como derrames pleurales masivos. Tipo de cáncer: cáncer de mama 20 (28,6%), cáncer broncogénico 12 (17,1%). Procedimiento quirúrgico: videotoracoscopía 53 (75,7%), pleurotomía 14 (20,0%). Se realizó biopsia pleural en 52 (74,3%). Complicaciones post-operatorias en 7 (10,0%) pacientes. Presentaron reexpansión radiológica al alta 62 (88,6%) pacientes. Necesitaron nuevo procedimiento de evacuación pleural 6 (8,6%) pacientes. Supervivencia promedio 8,3 + 9,5 meses (Rango 1-46), con un 55,7% de sobrevida a los 3 meses de seguimiento. DISCUSIÓN: La pleurodesis con talco es un tratamiento paliativo efectivo del DPN, con adecuada reexpansión pulmonar al alta, baja morbimortalidad y baja necesidad de un nuevo procedimiento de evaluación pleural, similar a lo observado en la literatura

OBJECTIVES: Chemical pleurodesis is an important therapeutic tool to control recurrent malignant pleural effusion. Among the various sclerosing agents, iodopovidone is considered effective and safe. However, in a recent study, ocular changes were described after iodopovidone was used in recurrent pneumothorax. The aim of the study was to evaluate the efficacy and morbidity of iodopovidone pleurodesis in an experimental model. METHODS: New Zealand rabbits were submitted to intrapleural injection of iodopovidone at concentrations of 2%, 4% and 10%. Biochemical (lactic dehydrogenase, proteins, triiodothyronine, free thyroxine, urea and creatinine) and immunological (Interleukin-8 [IL-8], VEGF and TGFβ) parameters were measured in the pleural fluid and blood. After 1, 3, 7, 14 and 28 days, groups of animals were euthanized, and macro- (pleura) and microscopic (pleura and retina) analyses were performed. RESULTS: An early pleural inflammatory response with low systemic repercussion was observed without corresponding changes in thyroid or renal function. The higher concentrations (4% and 10%) correlated with greater initial exudation, and maximum pleural thickening was observed after 28 days. No changes were observed in the retinal pigment epithelium of the rabbits. CONCLUSION: Iodopovidone is considered to be an effective and safe sclerosing agent in this animal model. However, its efficacy, tolerance and safety in humans should be further evaluated. .

Background & objectives: Chemical pleurodesis is an accepted therapy for patients with recurrent pleural effusions and pneumothorax. Iodopovidone has been shown to be safe and effective for chemical pleurodesis in several studies. The aim of this systematic review was to update a previously reported meta-analysis on the efficacy and safety of iodopovidone pleurodesis. Methods: Two databases MEDLINE and EMBASE were searched for a period (1952-2010), and studies that have reported success rates with iodopovidone pleurodesis were selected. The proportions with 95 per cent confidence interval (CI) were calculated to assess the outcomes in the individual studies and the results were pooled using a random effects model. Results: Thirteen eligible studies with 499 patients were included in the mata-analysis. The success rates varied from 70 to 100 per cent in different studies with the pooled success rate being 88.7 per cent (95% CI, 84.1 to 92.1). The success rate was not affected by the method (tube thoracostomy vs. thoracoscopy, 89.6 vs. 94.2%) or the indication of pleurodesis (pleural effusion vs. pneumothorax, 89.2 vs. 94.9%). The only significant complication reported was chest pain of varying degree. Systemic hypotension was reported in six patients across the studies. There were no deaths associated with iodopovidone pleurodesis. Statistical heterogeneity and publication bias were found. Interpretation & conclusions: Iodopovidone may be considered a safe and effective agent for chemical pleurodesis in patients with pleural effusions and recurrent pneumothoraces.

OBJECTIVES: To evaluate the effectiveness and safety of pleurodesis carried out entirely on an outpatient basis in patients with recurrent malignant pleural effusions and Karnofsky Performance Status scores <70. METHODS: This study was a prospective trial comprising patients with symptomatic recurrent malignant pleural effusion and Karnofsky Performance Status scores <70 but >30. All selected patients underwent pleural catheter placement (14 Fr) in an outpatient facility. When chest radiography revealed post-drainage lung expansion of >90 percent, pleurodesis (3 g of talc) was performed. Catheters were maintained until the daily output was ,100 mL/day. The patients were evaluated in the first month and every three months thereafter for fluid recurrence, the need for additional procedures, and complications. RESULTS: During the study period (January 2005 to July 2007), 64 patients (24 men, 40 women), with an average age of 61.4 years, underwent elective chest tube drainage. Primary sites of the underlying malignancy were breast (27), lung (22), and others (15). Sixty-six pleural catheters were placed (bilaterally in 2 patients), and 52 talc pleurodesis procedures were performed. Fourteen patients had a trapped lung and were excluded from the trial. No complications were observed during catheter placement or pleurodesis. Post-pleurodesis complications included catheter obstruction (4 patients) and empyema (1). The average drainage time was 9.9 days. The recurrence rate observed in patients that were alive 30 days after pleurodesis was 13.9 percent (5/36 patients). Six patients required additional procedures after the pleurodesis. The average survival time was 101 days. CONCLUSION: In this study, talc pleurodesis was safely performed in an outpatient setting with good efficacy and a reasonable complication rate, thereby avoiding hospital admission.

OBJETIVO: A pleurodese é uma alternativa eficaz no controle dos derrames pleurais malignos, mas existem controvérsias a respeito de sua indicação e técnica. O objetivo deste estudo foi avaliar como é realizada a pleurodese em países da América do Sul e Central. MÉTODOS: Profissionais que realizam pleurodese responderam um questionário sobre critérios de indicação para pleurodese, técnicas utilizadas e desfechos. RESULTADOS: Nossa amostra envolveu 147 profissionais no Brasil, 49 em outros países da América do Sul e 36 em países da América Central. Mais de 50 por cento dos participantes realizavam pleurodese somente se confirmada a malignidade no derrame pleural. Entretanto, escalas de dispneia e de status de performance eram raramente utilizadas para indicar o procedimento. Aproximadamente 75 por cento dos participantes no Brasil e na América Central preferiam realizar a pleurodese somente no caso de recidiva do derrame, e a expansão pulmonar deveria variar de 90 por cento a 100 por cento. O talco slurry foi o agente mais utilizado, instilado via drenos de calibre intermediário. A toracoscopia foi realizada em menos de 25 por cento dos casos. Febre e dor torácica foram os efeitos adversos mais comuns, e empiema ocorreu em < 14 por cento dos casos. A média de sobrevida após o procedimento variou entre 6 e 12 meses. CONCLUSÕES: Há variações consideráveis quanto aos critérios de indicação para pleurodese, técnicas utilizadas e desfechos entre os países. Talco slurry é o agente mais frequentemente utilizado, e a toracoscopia é a primeira escolha no Brasil. Os baixos índices de complicações e o tempo de sobrevida elevado indicam que a pleurodese é efetiva e causa poucos efeitos adversos.


OBJECTIVE: Pleurodesis is an effective alternative for the control of malignant pleural effusions. However, there is as yet no consensus regarding the indications for the procedure and the techniques employed therein. The objective of this study was to evaluate how pleurodesis is performed in South and Central America. METHODS: Professionals who perform pleurodesis completed a questionnaire regarding the indications for the procedure, the techniques used therein, and the outcomes obtained. RESULTS: Our sample comprised 147 respondents in Brazil, 49 in other South American countries, and 36 in Central America. More than 50 percent of the respondents reported performing pleurodesis only if pleural malignancy had been confirmed. However, scores on dyspnea and performance status scales were rarely used as indications for the procedure. Nearly 75 percent of the respondents in Brazil and in Central America preferred to perform pleurodesis only for recurrent effusions and stated that lung expansion should be 90-100 percent. Talc slurry, instilled via medium-sized chest tubes, was the agent most often employed. Thoracoscopy was performed in less than 25 percent of cases. Fever and chest pain were the most common side effects, and empyema occurred in < 14 percent of cases. The mean survival time after the procedure was most often reported to be 6-12 months. CONCLUSIONS: There was considerable variation among the countries evaluated in terms of the indications for pleurodesis, techniques used, and outcomes. Talc slurry is the agent most commonly used, and thoracoscopy is the technique of choice in Brazil. Pleurodesis is an effective procedure that has few side effects, as evidenced by the low complication rates and high survival times.

Introdução: A pleurodese com talco é o método mais popular para controle sintomático do derrame pleural maligno recidivante. A administração intrapleural do talco pode ser por videotoracoscopia ou dreno de tórax e qual o melhor método é uma questão controversa. Ainda que a expansão pulmonar seja um dos principais critérios de sucesso do procedimento, suas características pós-pleurodese ainda são pouco estudadas. Objetivo: Avaliar a expansão pulmonar após dois diferentes métodos de pleurodese com talco (dreno de tórax ou videotoracoscopia) em pacientes com derrame pleural maligno recidivante, considerando a evolução radiológica, diferença entre os métodos e correlação com desfecho clínico. O objetivo secundário foi comparar ambos os métodos quanto a: efetividade clínica, segurança, qualidade de vida e sobrevivência. Método: Estudo prospectivo randomizado que incluiu 60 pacientes (45 Mulheres, 15 Homens, idade média: 55,2 anos) com derrame pleural maligno recidivante entre janeiro de 2005 e janeiro de 2008. Os pacientes foram alocados em dois grupos: videotoracoscopia com insuflação de talco (VT) ou instilação de talco por dreno de tórax (DT). A expansão pulmonar imediata e a evolução da expansão pulmonar foram avaliadas através de tomografias de tórax obtidas nos primeiros 7 dias, 1, 3 e 6 meses após a pleurodese, as quais foram analisadas por dois observadores independentes. Efetividade clínica (considerada como ausência de necessidade de novos procedimentos pleurais durante o seguimento), complicações, tempo de drenagem e de internação hospitalar e qualidade de vida (questionários gerais e específicos) foram também analisados. A análise estatística foi realizada através dos testes de Qui-quadrado e Fisher para as variáveis categóricas e Mann-Whitney para variáveis contínuas não paramétricas. Introduction: Talc pleurodesis is the most popular method to control recurrent malignant pleural effusion symptoms. Two methods may be used to deliver talc into the pleural space: videothoracoscopy or talc slurry through a chest tube ; which is the best method is still controversial. Although lung expansion is the most accepted pleurodesis outcome variable, its features are poorly studied. Objective: To analyze and compare radiological lung expansion after talc pleurodesis performed either by videothoracoscopy or chest tube and correlate it with clinical outcome. Secondary endpoints evaluated were: clinical efficacy, safety, quality of life and survival. Methods: Prospective randomized study that included 60 patients (45 Female, 15 Male, mean age: 55,2 years) with recurrent malignant pleural effusion, between January, 2005 and January, 2008. They were enrolled into two groups: videothoracoscopic talc poudrage (VT) and talc slurry through a chest tube (TS). Lung expansion was evaluated through chest CT scans obtained in the first 7 days and 1, 3 and 6 months after pleurodesis. All examinations were revised by two independent observers. Clinical efficacy (considered as lack of new procedures during follow up), complications, drainage duration, hospital stay and quality of life (general and specific questionnaires) were also analyzed. Categorical variables were compared with the Chi-Square test or Fisher´s exact test for small samples. Mann-Whitney test was used to compare continuous non parametric variables. Survival curves were calculated according to the Kaplan-Meier method and Log-rank test was used to identify factors that could interfere with survival. ABSTRACT

Recurrence after thoracoscopic surgery for primary spontaneous pneumothorax is a lingering problem, and many intraoperative methods to induce pleural symphysis have been introduced. We analyzed the effects of chemical pleurodesis during thoracoscopic procedures. Between August 2003 and July 2005, 141 patients among indicated surgical treatment for primary spontaneous pneumothorax in two hospitals of our institution allowed this prospective study. The patients were randomly assigned to 3 groups: thoracoscopic procedure only (group A, n=50), thoracoscopic procedure and pleurodesis with dextrose solution (group B, n=49), and thoracoscopic procedure and pleurodesis with talc-dextrose mixed solution (group C, n=42). There was no significant difference in demographic data among the three groups. The two groups that underwent intraoperative pleurodesis had significantly longer postoperative hospital stays (A/B/C: 2.50+/-1.85/4.49+/-2.10/6.00+/-2.58 days; p=0.001) and a higher incidence of postoperative fever (A/B/C: 10.0/22.45/52.38%; chi-square= 21.598, p=0.00). No significant differences were found for recurrence rates or the number of postoperative days until chest tube removal. Therefore, the results of our study indicate that intraoperative chemical pleurodesis gives no additional advantage to surgery alone in deterring recurrence for patients with primary spontaneous pneumothorax. Thus, the use of such scarifying agents in the operating room must be reconsidered.

Se realizó un corte preliminar de un estudio descriptivo y prospectivo que se lleva a cabo en el Instituto Nacional de Oncología y Radiobiología, con el objetivo de conocer la efectividad de la pleurodesis con talco para evitar las recidivas de los derrames pleurales de causa maligna, aplicado a través de una pleurotomía mínima baja o por videotoracoscopia. Además, para valorar la ocurrencia de efectos adversos y complicaciones. Catorce individuos fueron incluidos tras presentar derrames secundarios a: metástasis de cáncer de mama (7; 50 por ciento), de pulmón (2; 14 por ciento), de colon (1; 7 por ciento), esófago (1; 7 por ciento), de ovario (1; 7 por ciento) de cuello uterino (1; 7 por ciento) y un paciente con mesotelioma (7 por ciento). No hubo recidivas sintomáticas ni efusiones mayores de 500 mL, tampoco efectos adversos menos graves ni graves. Por esta razón valoramos que en estos pacientes fue efectivo y seguro el uso del talco para provocar pleurodesis en los derrames pleurales de causas malignas y que mejoró la calidad de vida de estos(AU)


He/she was carried out a preliminary cut of a descriptive and prospective study that is carried out in the National Institute of Oncología and Radiobiología, with the objective of knowing the effectiveness of the pleurodesis with talc to avoid the relapses of the spills pleurales of wicked cause, applied through a low minimum pleurotomía or for videotoracoscopia. Also, to value the occurrence of adverse effects and complications. Fourteen individuals were included after presenting secondary spills to: cancer metástasis of he/she suckles (7; 50 percent), of lung (2; 14 percent), of colon (1; 7 percent), esophagus (1; 7 percent), of ovary (1; 7 percent) of uterine neck (1; 7 percent) and a patient with mesotelioma (7 percent). there were not symptomatic relapses neither gushes bigger than 500 mL, neither less serious adverse effects neither burden. For this reason we value that in these patients it was effective and sure the use of the talc to cause pleurodesis in the spills pleurales of wicked causes and that it improved the quality of life of these(AU)

ABSTRACT

Se define el neumotórax como la presencia de aire en la cavidad pleural. Se clasifica según su etiología, su condición fisiopatológica y su magnitud. El neumotórax espontáneo (NE) corresponde a aquél no causado por trauma u otro factor precipitante conocido, y puede ser primario (NEP), cuando el parénquima pulmonar es sano o con alteraciones mínimas, o secundario (NES), cuando existe alguna patología parenquimatosa pulmonar condicionante. Existen guías internacionales de tratamiento, a pesar de lo cual muchas veces el manejo es variable y no estandarizado. Se presenta una revisión de 50 casos de NE egresados durante 5 años del Servicio de Cirugía Adultos del Hospital Dr. Gustavo Fricke, realizando su perfil epidemiológico, exponiendo los criterios de tratamiento utilizados en nuestro hospital y comparando las formas de enfrentamiento con las descritas en otros centros. Un 66 por ciento de los pacientes se consideraron como NEP y un 34 por ciento como NES. La edad media de los casos fue de 37.2 años. Un 86 por ciento correspondió a sexo masculino y sólo un 14 por ciento al femenino. Como tratamiento de elección, se realizó una pleurotomía mínima (94 por ciento), de preferencia con tubos de grueso calibre. En 26 por ciento de los pacientes se utilizaron técnicas complementarias, como pleurodesis por tubo pleural, apicectomía del lóbulo superior, bulectomía o pleurodesis quirúrgica. En 10 pacientes se realizó toracotomía, en 2 casos una videotoracoscopía (VATS),y sólo en un paciente se realizó pleurodesis química a través del tubo pleural. Se estima necesario protocolizar el manejo del neumotórax y la selección de pacientes candidatos a resolución quirúrgica, como guía para cirujanos no toráxicos.

ABSTRACT

Although reports on pleurodesis date back to the beginning of the 20th century, the search for the ideal sclerosing agent is ongoing. Several agents have been studied and used, but talc continues to be the most popular. However, potentially harmful systemic side effects have been associated with talc pleurodesis. In this article we discuss the likely mechanisms of pleural inflammation and pleurodesis with emphasis on the systemic response due to the instillation of talc into the pleural space.


Apesar dos relatos sobre pleurodese remontarem ao início do século XX, ainda hoje se busca o agente esclerosante ideal. Diversos agentes foram estudados e utilizados, mas o talco é considerado o mais popular. No entanto, efeitos sistêmicos potencialmente tóxicos tem sido associados à pleurodese pelo talco. Neste artigo discutimos os prováveis mecanismos de inflamação pleural e pleurodese, com ênfase na resposta sistêmica produzida pela instilação intrapleural de talco.

O derrame pleural recidivante é uma situação clínica comum que compromete a qualidade de vida dos pacientes, em especial dos portadores de doença oncológica em estágio avançado. A abordagem terapêutica do espaço pleural é variada, incluindo procedimentos agressivos como a pleurectomia. A pleurodese é a técnica mais freqüentemente utilizada, podendo ser induzida tanto através da inserção de cateteres pleurais, como por procedimentos cirúrgicos amplos (toracotomia). São vários os agentes esclerosantes indicados, incluindo o talco, que é o mais utilizado, o nitrato de prata e recentemente as citocinas proliferativas. Este artigo resume as principais abordagens do derrame pleural recidivante e particularmente da pleurodese, suas indicações, vantagens e desvantagens para a aplicação na prática diária do pneumologista.


Recurrent pleural effusion, which is commonly seen in clinical practice, compromises patient quality of life, especially in patients with advanced malignant disease. The therapeutic approach to the pleural space involves a wide range of techniques, including aggressive procedures such as pleurectomy. Among such techniques, pleurodesis is the most frequently used. Pleurodesis can be induced through the insertion of pleural catheters, as well as through major surgical procedures (such as thoracotomy). There are various recommended sclerosing agents, including talc (which is the most widely used), silver nitrate and, recently, proliferative cytokines. This article summarizes the principal approaches to the treatment of recurrent pleural effusion, pleurodesis in particular, addressing the indications for, as well as the advantages and disadvantages of, their application in daily pulmonology practice.

PURPOSE: Chemical pleurodesis is a therapeutic tool for the treatment of recurrent pleural effusions, mainly those of neoplastic etiology. In the past, tetracycline was the sclerosant agent of choice in clinical practice, but presently, there is no consensus about an ideal agent. The aim of this study was to evaluate the effectiveness of macrolides (azithromycin and clarithromycin) or quinolones (levofloxacin and gatifloxacin) in inducing experimental pleurodesis in rabbits. METHOD: Forty New Zealand rabbits randomized into groups of 10 received (at a total volume of 2 mL for each animal) 1 of the 4 drugs by intrapleural injection. After 28 days, the animals were euthanized and the pleural cavity was evaluated macroscopically and microscopically. RESULTS: The intensity of the macroscopic adhesions was mild in all groups. On microscopic analysis, minimal pleural fibrosis and inflammation were observed in all animals. CONCLUSION: The macrolides (azithromycin or clarithromycin) and the quinolones (levofloxacin or gatifloxacin) when injected into the normal pleural space of rabbits are not effective in promoting pleurodesis. Additional research is required to identify sclerosing agents capable of inducing pleurodesis.


OBJETIVO: A pleurodese química representa uma ferramenta terapêutica utilizada no tratamento dos processos pleurais recidivantes, principalmente nos derrames neoplásicos. A escolha do melhor esclerosante pleural é ainda motivo de controvérsia, não havendo consenso com relação ao agente considerado ideal. O objetivo deste estudo é avaliar a efetividade dos macrolídeos (azitromicina e claritromicina) e das quinolonas (levofloxacina e gatifloxacina) na indução de pleurodese experimental em coelhos. MÉTODOS: Quarenta animais randomizados em grupos de 10, receberam, em volume total de 2 mL, estas drogas através de injeção intrapleural. RESULTADOS: Após 28 dias, os animais foram sacrificados sendo avaliada a cavidade pleural. A intensidade das aderências macroscópicas assim como da fibrose e da inflamação observadas à microscopia foi discreta tanto no grupo que recebeu macrolídeos quanto naquele que recebeu quinolonas. CONCLUSÃO: Azitromicina, Claritromicina, Levofloxacina e Gatifloxacina quando injetados na cavidade pleural de coelhos, não são eficazes na indução de pleurodese. Novas pesquisas devem ser realizadas com o intuito de identificar agentes esclerosantes capazes de produzir sínfise pleural.
